I did Maw with my thief after the daily reset and didn’t receive the bonus chest. It wasn’t until I did it with my warrior a second time, before the next reset, that I got a bonus chest.

This also happened before a couple days before with my warrior on wurm, and I had to do it again with a different character to get the bonus chest.

If you’re camping, you don’t get the chest. You’ll need to rotate or get out of the map if you plan to event-farm.
I would link you a post with the studied conclussions after some testing on boss chests, but threads get lost way too quickly by people that open new ones for things we’ve alredy reported (like you just did!).

You can only get the bonus chest once per day per event account wide, plus you have to be lv40+ to get the bonus chest. The World chest you should still get everytime (though in my experience you get more and more crap stuff the more you do it.) I mainly redo world events if I’m levelling a toon as it gives a decent amount of exp.
