I have dynamic IP and have still never once had to enter a new code.

I don’t know what’ up at your end but I only ever have to enter the code if I reset my router or something. I suspect it’s an issue with your ISP tbh.

Yes probably OP has some internet service that change IP many time. This happen to some people depend on who they have for internet. With out this I am surprise how many people insult OP for browser because this is off topic and not about their problem.
I had to get rid of it too. When I’m tethered through my phone, I need to get a new code each time my phone gets a new ip. My location doesn’t have much of a signal, so my phone losses connection and gets new ips all the time. It drove me crazy for the short amount of time I used it.
Umm, I believe you are a victim of DNS hijacking. The toolbar to the right of the Yahoo toolbar appears similar to a certain malware that redirects your web address lookups through the hackers’ servers.
Remove whatever toolbar that is, and then immediately restart (BEFORE removing any others). If it reappears after the computer restarts, it’s malware. If not, I’m wrong.
