As it stands, when you clear a dungeon in explorable mode (all paths), there is no tangible reward. I think a kewl way of rewarding doing all of the paths of an explorable mode dungeon is to make it a monthly achievement.
Here is how it would work:
The achievement would be for each of the dungeons on a monthly basis.
If you complete all the paths of a particular explorable mode dungeon, you are rewarded with a random piece of exotic armor or weapon from the dungeon set + any other misc rewards is seen fit including xp, karma, mystic coins, etc.
The monthly nature of this achievement would prevent speed run farming AND it would give everyone an incentive to play all the paths of all the dungeons at least once a month.

Happened for the first and only time to me yesterday.
Restarting the game fixed the issue.
