A:
Recipes for named exotics like Mjolnir work 100% of the time.
Certain recipes, such as Mystic Clovers, have a certain chance to yield mystic clovers and a chance to yield other materials.
Other recipes, such as material promotion always work, but give a variable amount of the resulting material.

Don’t quote me as I am not 100% sure, but I believe that fixed recipes have fixed results (unless it is a mystic clover or material upgrade one).
If someone gathered the 350 charged lodestones needed for Mjolnir and then it didn’t work…. Well I don’t think I’d sleep soundly being the guy who programmed that into the game

I’m pretty sure that fixed recipes have a 100% guaranteed result. For example take the recipes for promoting cores to lodestones. Or the ones for mystic weapons. They always yield the expected result.
This is how I imagine the guy who controls the roll tables for the mystic forge and salvaging:
For the fixed recipe’s there is supposed to be a 100% success rate. It’s only when when you dump in 4 random item’s that it has a possible chance to screw you over.
