Do you only get the one box with the light, med and heavy helm per account?
Or can you get more by staying for another fireworks show?

Yes just once. I was at the fireworks yesterday and got them. I was there today as well and you don’t get them twice.

Since it is tied to the achievement like the south sun back pieces i believe you can only get them once.

Not at the same time, but they’re account bound, not soulbound, so you can move them back and forth if you want to.
