Guild Wars 2

During my fourth attempt at this achievement I was partied with a full guild group on my main server. When we downed the second knight, two members of my party received credit for this achievement however myself and the other two did not. Several others in the zone also received credit before all three bosses were dead. Since this achievement is a huge dps race it is unfair that it is being trivialized by some bug which is erroneously rewarding credit for this achievement.

On a similar note, on our TTS overflow (many others can attest to this), we killed all three with 4 seconds to spare, but none of us got the achievement. Something is wrong.
