Exploding unknowns in Temple of Grenth event
Well, I found that the culprit is Risen Plague Carrier, which shows as “Unknown” in chat window during its explosion.
If you’re facing level 84 mobs, this is somewhat hard to avoid during the Temple of Grenth event due to heavy culling issues, causing them to spawn and explode in an invisible manner.
The best tactic is taking down Veteran Risen Acolytes immediately before they spawn various types of the risen, including Risen Plague Carrier.
Those veteran risen acolytes seemed to get that summoning ability in a previous patch and it put a number of events out of balance, including one of the personal story missions for claw island. This change came in at the same time as the risen krait changes and in both cases there doesn’t seem to have been enough analysis/testing of the new abilities. The new AC explorable cave troll was also inherited by AC story, where he definitely doesn’t belong, so this isn’t an isolated problem.
This is it! This is what’s currently causing all the failed Grenth attempts since the patch! They recently increased the scaling for the Orrian events and now protecting Jonez is almost impossible. The enemies seem to be hitting for double that now, at least.

Not really this alone. there are never more than 1 or 2 Acolytes, each summoning 2-3 Carriers, each of which can hit max. 5 targets afaik. That doesn’t make much of a difference with 100 players especially since they only hit in melee. On the other hand, those grouped up with all the other insanely buffed mobs (read: spiders), cause serious issues, and kill most of the downed players almost instanteniously.
I’ve also experienced this issue (I was redirected here thanks to some helpful posters).
I think what is causing a lot of these wipes, is that multiple of these plague carriers explode in a row. For example, one plague carrier explodes and downs you, and a fraction of a second another one explodes, thus instantly killing you. This makes it seem as if you are not downed at all, but instantly killed. My chat log seems to confirm this.
Along with the culling issues, this makes it impossible to see these guys coming.

Are there plague carriers at the Melandru temple as well? I’ve seen the same thing in the combat log there but I don’t recall ever seeing a plague carrier there. I think it might be the rotmouths.

I can think of two instances where a veteran acolyte has been there, which spawns them, so yes. If you were just in the one on TC he probably didn’t outlast the rotmouths because some of us are sick of the invisible nukes and make a point of wiping these guys off the planet as soon as we spot one.

I definitely wasn’t there, I’m on an EU server.
It would explain the randomish nature of the attacks though.
There is always at least one Veteran Risen Acolyte at the final Melandru event. I’ve run this temple many times and never not seen one.
Veteran Risen Acolyte appears in several locations (if you’ve seen them elsewhere, please contribute!) It can spawn mobs and more until there’s a dozen — I don’t know the max, but all the mobs will despawn upon killing the Veteran Acolyte.
