How do you open the Mad King's Realm doors?
That is a very good question actually.
Usually to me , i go to a point where they appear , and usually a normal door pops up.
I beat the usual mobs that come with it , and later the Mad doors open so i can go there.
Im not sure if this is the right way , or if is actually someone on another part of the map doing something, but always work for me.

I just spent like 25s going from all the potential areas to find one and guess what, none are there…

There is an event that pops up in those three zones every so often, you will see it on your map, when the event is completed all the doors pop.
That’s not happening any more. I was in Queensdale for over and hour and people in the map were talking about how they’d been there for four hours. The doors are not opening any more. I was playing when the event went live and all the doors would opening, that’s not happening any more. I suspect the event chain has bugged and you can’t get in any more.
