Surely these Points of Interests are named for a reason. They are not just quest hubs or vendor markers. A lot of them are pretty cool hangouts, and places that clearly has a history unto itself.

I think that proper back story should be added to each place. This is done by talking to an NPC who knows the story for the place (maybe multiple NPCs for some place? Make them a small investigation quest?). After you learned about a place, the back story can be accessed at any time in the map screen.

I am wholly on board with this idea. I am the kind of player that read many of the books in the elder scrolls games, I love the little mini stories specific to different fantasy worlds. Malchior’s Leap is just such an example. This story is told through scrolls in a pact camp. It would be fun to find scrolls for different POI’s in the vicinity, and encourage active clearing of mobs instead of just running through for map completion.
