One thing that I’ve noticed throughout my travels is that completing heart missions doesn’t really feel satisfying to me. I pretty much only do them to get the silver and completion (and I do the completions to also get silver…). But the quests themselves don’t really seem to matter. For instance you might go around setting traps or stomping grub holes, but at the end of the day nothing comes from doing that.

So, I would like it if each heart mission had an event that spawned when you complete the heart. To keep the event from spawning repeatedly over and over from multiple people turning it in around the same time, maybe there could be a timer where after the event spawns it can’t spawn again. But some examples could be something like where if you set traps that a swarm of enemies comes at you and the hearty NPC and the traps actually are used to trap them. Or in the other example if you stomp on grub holes then maybe a giant veteran grub will spawn and try to ruin your day for stomping the grub holes.

You may say that tying events to hearts could be bad because you could only do them once. However, even if you complete a heart you can still partake in what the heart originally asked you to do. So you can still set traps and stomp grub holes. There could be an invisible heart meter that fills if you already completed the heart that, when it gets full, will proc the event (if the timer has ended of course).

This also gives players the power to spawn events instead of just wait for them. Which you may consider to be good or bad, but I consider it to be good.
