Do you have to do an entire tier in one run?
You have to do all 3 (4 if even) in one run to advance.

The fractals are randomized; I don’t think I’ve seen the same one pop up twice in a single fractal run. They cannot be chosen. You have to do 3 every odd level and 3+bonus fractal every even level in one zoning order to advance.
I assume this is a measure to stop people from repeatedly resetting the fractal set until they get an easy one to burn through and just accruing credit that way, but it really hurts with the current disconnect=permaboot issue.
That said, I’d stay away from using “tier” to describe a set of three fractals. “Tier” is fast becoming common language for a range of 10 fractal levels that describes the general difficulty and ability to get a daily from that range. Level 1-9 = Tier 1, Level 10-19 = Tier 2, etc.

Try planning a premade guild group to meet at a certain time. Being able to log on and be inside a fractal in <5 minutes helps a lot with knocking them out in a short play session
Depending on your group and the fractals you face, the time to complete a set of three can vary greatly.
I was in a group that spent nearly an hour trying to complete the frist part of the swamp. I’ve also been in a group that completed it in less than 2 minutes. How much time can you spare? If you have an hour, doing a set of 3/4 fractals is entirely posible, however it may be difficult to do in that time with certain groups.
Also, the 3 fractals have do be done not only in a single trip into the fractals area, but without returning to the staging area—that means no armor repair or vendor (unless you bring those services with you). This may or may not be news, I point it out because of the potential ambiguity.
