If you did either of those, most likely yes.
A few have been “fixed”, some have probably gotten actual fixes, and probably some I haven’t experienced personally.
Auto-casting Scepter auto-attack can break dodge sometimes too.
It was auto-attack dagger (necro). Come to think of it, it’s happened in the past as well, when I had no lag — I just never thought about it much. But theoretically, it shouldn’t be possible to get frozen mid-dodge, right?

Auto-casting a fast spell can sometimes cancel dodge.
Weapon swap can also cancel dodge.
And yeah necro doesnt have any whirl anims which would cause fumble but despite this i have experienced fumble dodges on lupi solo before. I assume it was because of lag though.
Fumble dodges also just randomly happen. A fumble dodge isn’t exactly a dodge where you do not get evade frames, but a dodge where you “fumble” interrupt the dodge midway. This can happen at various points but it will always consume endurance like normal.

How far into the roll are we talking here? Dodges definitely have startup. It’s most likely due to latency. Since dodges seem to animate immediately upon pressing the button, you’re probably just lagging out and it’s showing the dodge only on your end.
Next time you get frozen by that boss try to find out what you look like to the others in your party.

Do you think I’d be able to tell whether it was lag by the endurance bar (I’m always solo in this dungeon, so no party to help me look)?
If it was lag, my guess is that I’d see the (client-side only) dodge animation and (client-side only) endurance loss. But once communication with the server is updated, the endurance should shoot right back up since I didn’t actually use it, right?
Honestly I’ve never fumbled from those notorious attacks since they got patched. Sometimes the dodge will roll on the spot, but I’ll always get evade frames.
Though lag gets the better of me more frequently, e.g dying to Lupi’s swipe at the end of my dodge roll, too far away for the hit to even connect.
Looks like guard GS3 tends to give some trouble too. I got smacked in the face by lupi earlier today when i wanted to dodge right after it.
