So here’s what happened – myself and 4 others were doing Ascalonian Catacombs (path 3). One of our members disconnected. We removed that person from the party. I then went to Lion’s Arch, leaving the instance, to recruit another 5th person. When myself and the new person returned to AC to try to re-enter, it put myself and the new person in a brand new instance of AC each time…we could not rejoin the party who had already progressed.
