Open Grave Achievement .
As far as I know, that achievement is for Solid Ocean fractal (Jade Maw). You have to kill all mobs (also white mobs) on the way to Maw.
Aquatic ruins has different achievement in which you have to skip the Krait as a dolphin.
Open Grave is for the Solid Ocean (Jade Maw) fractal, not the Aquatic Ruins one.
