SE p2 escorts event can bug out after killing the 4 champions but it is easily preventable by killing the 2 elite mobs that accompany the champs before the last of the 4 champions is killed.

From what I’ve seen Tzark will bug if he dies while talking to start the burrow event. You prevent it by running all the way to the back of the burrow room, so the 4 mobs on the stairs deaggro.
For TA up & fwd, the paht bugs out if you choose another option that starting the path when talking to Morrigu at first, then after choosing the 2nd or 3rd option, selecting the option to start the path.
Also afaik, if more than 1 person talks to path picking NPC at same time it bugs.

My workaround for this, usually, is telling the whole party to move into the secluded room where the second burrow spawns, this way Hodgins magically spawns at the table.
i heard that if you (in the molten fractal) let frostbite stay and fight enemies but rox and braham runs towards the gated area, you will bug the run and the doors won’t ever open
but there are people who will help you get back up when you are downed

We had him bug for no reason. We then killed him on the stairs to try and fix it. He would just follow us around the room not triggering anything.
This was done after the tunnel trigger after scepter pieces didnt activate in our all path in one run. So we decided to do legit P1 to finish. Exploit it and it breaks. Do it legit and it breaks.

For CoF path 2 : Magg teleports to people at the waypoint instead of the magma stone
I’m not 100% sure, but I think that Magg teleports to people at the waypoint when the last person to waypoint is out of the room where you did the explosives event. If the last person to waypoint is still in the room, it should be ok. Where the room start exactly is hard to tell, but it seem to bug even if the last person to waypoint is in the hallway near the chest.

Not Randall. The agent from order of whisper who is in charge of opening the gate.

CM P2: Running into Taylor’s room too early will bug the path and prevent being able to continue. Happened a lot during our record attempts, is very easy to reproduce simply by running into the room quickly. (Not sure what causes it, probably some dialogue the npc has to start.)
AC Story: Not 100% sure if thats the reason, but triggering multiple cutscenes (during the event where you have to kill Kasha, nente and the 2 lovers) at the same time may prevent Kasha from getting attackable. Didn’t find a way to fix this so far.
I don’t think that’s what causes Lyssa to bug. During duos when I’m not with a thief I’ve many times just jumped up in the corner and waited for both randall + the whispers agent to die to the abom/trash mobs before killing them and have not had Lyssa bug.
What I thought was causing her to bug was someone approaching the statue without the NPCs there, or possibly the whispers NPC getting killed by the champ spider + elite spiders while her dialogue sequence is triggered. It could be one or the other or possibly both but I’m 100% sure that her dying to the abomination after melandru does not bug her.
In the Cliffside fractal, if an engi swaps kits while holding the hammer it will disappear forever and never respawn, forcing you to restart the whole fractal.
In Arah path 2, sometimes Belka’s cut scene never triggers and the instance has to be reset because she stays green. This seems to happen more when some people are still running past the trash mobs when someone activates her. Maybe it has to do with whom the NPC is following?
Not dungeon halting but annoying, also in Arah path 2, there are many spots that running into during the Alphard fight will cause her to reset to full hp. One in particular is the bone wall. The main preventative measure here is to make sure everyone is in the arena before aggroing her.
Speaking of that bone wall in Arah path 2, if the party wipes fighting Alphard the bone wall will often remain. This can be fixed by having the whole party die at the same time.

Well, all members just have to die(can be different time(tested)) and wait a bit after but still annoying thing.
I have a follow up on that weird Alphard glitch where she resets to full hp. I was testing around with it and I’m pretty sure that it happens when she does her scorpion wire attack, because as you all know it targets everyone in the party, yeah?
Well, what I think is the problem is that her hook targets a player that’s out of bounds for her (behind bone wall, etc) and it causes her hp to just immediately go back to full. I’m not 100% sure on this so maybe someone can confirm it.
I believe the alphard reset glitch is caused by the entire party getting OOC – most likely when she drops a bomb just before teleporting and everyone dodges back. I’ve gotten OOC without moving when she teleports sometimes too. It should be avoidable by making sure to not be OOC for more than 2 seconds – easily done by fighting near the boxes instead of the normal PUG spot.

SE 3 P3 clown car bugs if somebody disconnects or leaves during that part (I think this is the cause).
CM p2 taylor bug is easy reproducible if you go into the room where she spawns before the door on the ground floor opens she doesn’t spawn.

Then there’s the bug in Thaumanova where you become completely invisible as a character. Can’t even see your weapon so you have to use spell effects to gauge your relative position on the floor the boss is in.

Have had it happen while we were all in the PUG spot, doesn’t really disprove the theory (might be out of range, or maybe it’s targetting a phantasm or something htat happens to disappear right at the wrong time), I’ll try to keep an eye out for it.

I think it is from using the teleport devices to exit the heat room and the cannon room.

This happens to me 100% of the time. I usually solo heat room but i thought it happens to everyone in party, but maybe not. We’ve fixed this by logging out and logging back in but i’m used to it.
So I did Thaumanova today 3/31 at lvl 34 fotm… I solo’ed heat room and guess what? I didn’t go invis. I even said in party chat that it was very weird.
New Bug (for me at least): We teleported to chest room, all got our loot without any delay, and then all readied up. We got the “beam me up scotty” animation but then we didn’t go anywhere. We just stayed in the chest room… we tried relogging and it brought us back to the very first room with the detonator. The wall was intact so we were stuck in there. Briefly I heard one of the quest dialogues but that’s about it. No clue why it could’ve happened but there were no wipes at any part and we weren’t skipping mobs or anything. Was actually a very smooth run. 2 days in a row i get stopped midway into a fractal. This was more frustrating because it was right before the boss fractal… ughhhh. They need an option to restart level if they’re going to have so many kitten bugs.
