Honor of the Waves is bugged for Butcher route. The boss happens to reside in a room that is currently has visible, but unsteady floor.
You come to Butcher (event starts), and fall down through floor. Probably boss falls through floor as well. I don’t know for sure as I’ve never seen him today.
Needless to say it’s impossible to finish this route.
Unfortunately my friends don’t want to repeat this pretty dungeon after this happening, so I don’t know if you can’t swim to other possible victims of “dungeon buffs”.

Is this a new bug? I have killed The Butcher 30+ times and never had a problem.

Yes. This bug was introduced in recent patch (18 sep).
