Are the weapon and backpack containers for the 5th year anniversary meant to be once per account or per each character turning 5?

They are tied to 5th year achievement so you can get them once per account.

Just opened the gift on three different characters…only one got the skins. Also, those came in a a pop-up chest rather than directly from the gift package. So I assume those are meant to be per account.

Correct. The weapon/backback skins, wardrobe unlock, and lounge passes are once per account. The dye selection, karma, birthday booster, experience scroll, and teleports to friend are per character.

Mine appeared when I opened my birthday gift.
