Looks like character based unfortunately. Really punishes players who remake characters and those who would routinely create pvp characters.
A Birthday Gift is an item received by a character once the character reaches their birthday; each year after the day of account creation. For example, a character created on August 28th, 2012 will receive their birthday gift on August 28th, 2013.
It’s buggy, too. I have numerous characters that were created the first half hour of the first day of the headstart, and I have still not gotten my birthday present. :|

Same. Actually I still have 3 characters which were created within the first 5 minutes the game went live (headstart), but none of them received anything.
All my characters /age says 11 months, but for account it says 12 months.
The mail you get (according to wiki) says this: “Congratulations! It’s the anniversary of your account activation”
I’ve seen others with the mini Jennah though.

This appears to randomly be character-based. Each of your characters might or might not get one, at random. So RNG applies.
Your last sentence should read, “For example, a character created on August 28th, 2012 might receive their birthday gift on August 28th, 2013.”
They usually roll out large amounts of mail like this in groups, so as not to overwhelm the system I guess. I only just got mine an hour ago, your time will come.

They’ve really muddied the water on this. We only get the gift on each character that is one year old, but the message reads “Congratulations! It’s the anniversary of your account activation…”!
