Does anyone know how to get this outfit? is it only available if I order the heart of thorns expansion?

it is not related to having the expansion or not

you will just have to wait until it comes back to the gemshop during a sale
so just check the gemshop daily while going on with playing normally
not much else that you can do about it

The monstrosity will be back in the gemstore eventually.

It was last available for 7 days starting March 7th, a month ago, so it might be a while now before it shows up again.
