Heavy bag issue
Honestly, since I’ve owned a Copper Fed Salvage-o-Matic I’ve not encountered any of the problems you face.
Salvage whites, blues and greens with the Salvage-o-Matic. Hold onto 1 or 2 Masters/Mystic Salvage Kits for the occassional rare that is worth more to salvage than sell.
Go by merchants every so often to sell any junk or green sigils/runes that you might’ve picked up.
Deposit any materials by clicking the cog in the top right corner and click “deposit materials”.
Remove any items that you don’t use frequently in your inventory. Store them in your bank. The items that you might use, keep them at the start of your inventory (or in an invisible bag) so that you can compact your inventory and they won’t get moved about.
Having infinite gathering tools is also useful as it saves a few inventory spaces that might be used on extra mithril/orichalcum tools and spares.

I have the salvage machine but I never salvage stuff that turns into ore or wood (this only applies to high lvl stuff that turns into mithril or elder wood) as you get more money if you vendor them.
I have mystic salvage kits for special needs (they stack 250 instead of 25 so I save a lot of space).
I admit I’ve mentioned a little too many details, and that may have mislead you. So my most irritating inventory issue I have is this: weapon switching (see details above). So if you (or anyone else) can tell me if exotic fractal bags work with ascended gear as well or not, would be awesome.
I was actually talking about this yesterday in the Players Helping Players section, how to make the best of the bag slots. I made a suggestion for those with x8 20 Slot Bags, but for those with x5 <18 Slot Bags, If anything, I’d just say go with, in this order:
1. Starter Backpack (for potions, tools, ect.).
2. Invisible Bag (for personal weapons and armor).
3-4. x2 Equipment Boxes (for weapons and armor you want to salvage).
5. Craftsman’s Bag (to just keep all the sigils and ascended mats out of the way).
The only thing you wouldn’t be able to really take of is junk, which would land in Starter Backpack, but if you go to merchants often it won’t be a problem. If you reaaaaally need more space for salvaging gear, I’d get a third Equipment Box and swap it out with Craftsman’s Bag.
Also, exotic fractal bags do not work with ascended gear. These bags only work for the rarity that they specify, which I feel is wasting potential space in regards to the exotic ones. There really isn’t much occasions you’ll have 20 exotics at any given time.
About the gear issue, of your weapons appearing outside the bag, I’m assuming is a bug with the bags as I have to deal with that as well, my offhand weapon just goes to my Equipment Boxes. It’s annoying at best, I just go find it and place it back in Invisible. I used to use x5 Leather Bags with no special qualities and I had to go find my personal weapon at the bottom of the bag all the time, I eventually just got used to it.

I have one invisible bag for personal items and then everything else clutters the rest. When I get full, I hop into WvW, sell/salvage/bank everything, and then hop out and continue where I left off. I will also restock gathering tools/salvage kits in WvW if needed.
I use the following setup on all 8 alts:
Starter
Craftsman
Equipment
(Any)
Invisible
I keep anything i use frequently for gameplay in the top row of starter bag. Ie: shovels, bandit keys, lockpicks, extra harvesting tools (if I am not running the toon with my infinites). This leaves the second row to collect junk items automatically, since I have a crafstman and an equipment box.
Personally I find having both the craftsman and equipment is wonderful for keeping things sorted without any intervention from me until one is overflowing.
The fourth bag is any generic bag, 18 or 20 that I have collected over the last few years, and I store any ‘personal’ stuff that can’t be vendered. Tonics, tools, toys, Teleportation Guns. If it can’t be sold, no purpose in wasting space in the invis.
I have one fractal box and found it incredibly annoying. No matter what you do, only accepting a specific rarity isn’t good enough. If they were “Rare or better” that would be awesome. But in general, the three rares I get before my equipment box is full are rarely hard to find, and the entire bag is full several times between exotic drops, so that box is worthless too.
As for your specific issue with changing gear, primarily I handle it manually. Usually swap a 2h for another 2h if possible. My engie I switch between pP/S and riffle often, so I just immediately grab the shield out of the equipment box and put it back in the invis. I have a specific order I keep all the weapons on each toon, so I am particular about putting them back where I want them.
The ONLY other solution is to have the equipment panel and the inventory open at the same time, and get in the habit of unequiping the offhand manually, by dragging it into your invis BEFORE swapping in the 2h. This at least puts every transfer under your control.
I would really like it if there were boxes like the fractal boxes which would accept a set rarity or higher. Have a Rares or better box, or an exotic and better box. And offer them up as Invis options as well. This is pretty common issue I think, its just one that has a simple “get used to it” fix. Would still be nice to see a better implementation.
Though, I am still holding my breath for a ‘toolbelt’ micro inventory to keep a few items at hand at all times, too. Sometimes I like to “Think” I am batman with all my tools and toys, but really start to feel like some hobo with an overflowing grocery cart.
