Is it possible to have an old item refunded?
You could contact the CS Team via the ‘Support’ link above/below, but it is possible they would need an approximate time indicating when the item ‘disappeared’ to replace it.
There is this policy, though: Please note that skins or partials skins that you previously purchased but which you no longer possess will not be unlocked on your account. The Guild Wars 2 Support Team cannot unlock skins that fall into this category. No idea if the policy has changed, or if it applies to skins acquired through other means than purchase.

ANet is pretty good about this, I misinterpreted a red post on the first halloween, had a ghastly grinning shield drop, and then because of the misinterpreted post I thought I needed to use it to not lose it, and soulbound it to a character. And in the end found the developer was talking about a different set of temporary weapons available at the time with the same skins.
And when I contacted support about it to have it replaced (because I didn’t want the skin, I wanted to sell it at a later date) they said they would love to but they didn’t have the tech yet for replacing items.
So I banked it, finally got around to asking about it again nearly two year later, and support replaced it for me without any trouble.

I get what you are saying but, as far as i am concerned, i need the original Advanced Spinal Blades in order to complete the recipe for the Tempered Spinal Blades.
How would they refund it though? It’s not a gem store item, so not something they can refund back to you. (they don’t refund in game gold)
I think the OP meant to say ‘replace’, rather than ‘refund’. =)
The change of the transmutation system didn’t delete or change the quality (ascended, exotic, rare, etc.) of any gear, as far as I know. I would double/triple/quadruple check every character’s inventory (use the search bar at the top to type in the name), and then check the bank (use the search bar there, as well).
When I can’t find an item, I use Guild Wars 2 Efficiency
With your API key (instructions for making one available on site), head to Account → Overview and then filter by type/subtype. That will show you all the packs you still have, whether equipped or stored. You can mouse over each to see if one of them is the transmuted version of the one you seek.
+1 on the you should use efficiency to search for the blades first
I deleted all of my characters (along with 9 legendaries) ~7 months ago since i thought it was a good idea at that time – don’t ask me why. I’ve changed my mind and got all of my characters and items recovered after an awesome experience with one of the GMs.
The support in this game is top notch so i think it shouldn’t be a problem for them to recover a single item.
