Is there a way to reach Underworld in GW2 ?
I’ve wondered this as well – from what I understand, not normally, no…however, a part of Underworld is available during the Halloween event.
I think this is right, but someone can correct me if wrong.

The Mists is the realm of the afterlife and the proto-reality that exists between the worlds which in turn are the building blocks of reality. They are the home to various gods and other powerful entities and the residences of various afterlives. The Mists resonate from the worlds around them, forming bits of their own reality — islands of existence that reflect the histories of their worlds
The Underworld is the realm of Grenth. Many years ago, the realm was ruled by Dhuum, a cruel and unjust god of death who was eventually overthrown and imprisoned.
When the body dies, the spirit will often move on to the Mists, usually the Underworld – though there are cases where spirits end up elsewhere due to external forces.
The Mad King’s Realm, also known as the Mad Realm, is the home of Mad King Thorn and a sub-section of the Underworld. It is divided into four sections and follows a Halloween theme in appearance. It is described as a place of death and tragedy. This zone is only available during Halloween events.
Tl;dr:
Mists is the in-between of parallel, non-parallel, everything in between, and more, of Worlds. (Yes everything in between the in between.)
The Underworld is a subsection of the Mists.
The Mad King’s Realm is a subsection of the Underworld.
