I followed this guide and collected the maps, I got maps 1-2-4 on the first try but didnt get 3 with 3 tries, is this possible that because I picked it already (with a old character which is deleted now [with the item]) I cannot pick it anymore?

You don’t only get one. It’s just RNG.

Even after you have them all you still keep getting map scraps.
