I have a Total Makeover kit sitting in my bank. If I use it on my Beta Revenant is it gone forever?

Nope, your beta characters have a copy of your bank.
No changes you make on a beta character will change the original.

Easiest way to see, log onto beta toon, take kit out of bank, switch back to original toon, it should still be there.

It will be consumed only in the beta, in your regular characters it will stay the same.
To make sure before you use it, pick the total makeover from the bank but don’t use it, log in a normal character of yours and the makeover will still be in your bank, if that happens then it is save to use in beta.
