Salvaging Ascended Gear

Does anybody know the rate at which inscriptions/insignias are recovered from Ascended gear? And are upgrades recovered at a 100% rate?

I salvaged about 10 ascended weapons and about 15 ascended armors and ended up with 1 insignia. the rate is not very good.
yes, all sockets and infusions were returned.
