I propose a title system similar to the GWAMM title track from Guildwars 1.
The key difference being instead of each title being +1 an experience bar is used with different titles giving different values.

Title values
Each title would be assigned a value, say from +1 to +8 which would progress your exp bar to the next title level.

Example:

Easy titles (+1):
Loyal
Respected Achiever
Honorary Skritt
Gladiator
Combat healer
Traveler

Medium (+3)
Dungeon Master
Fashion Collector
Champion x

Hard (+6)
Dungeoneer
Heralded Achiever
Champion Brawler
The Sunbringer
Legend of the Mists

Elite (+8)
The Blazing Light
Legend of the Arena
GWAMM

The experience Bar
Uses a standard leveling system to go up, say
5 points to level 1.
8 points to level 2.
14 points to level 3.
20 points to level 4.
30 points to level 5. etc etc.

Reward System
Why are you leveling this up?
Each new level unlocks a new title (that does not count for points).

In addition to this advancing the title system unlocks color changes for your guild,title and game as displayed to other players (optional you have to select the color you want).
I.E:
level 1: white
Level 2: Blue
Level 3: Green
Level 4: Yellow
Level 5: Orange
Level 6: Pink
Level 7: Purple
Level 8: Rainbow.

Questions
Q: Why do we need this?
A: Its a nice progression system and gives titles a clearer role, it also helps clear up confusion when nice sounding titles (The Annihilator) are given for doing nothing you can still identify a player who has unlocked a lot by their title colour.

Q: What about previous/unobtainable titles (HOM and LW and old arena titles)
A: They should count too, this is a prestige system for veterans and all previously earned titles should count.

Q: LW titles will eventually be abundant and skew this title track thoughts?
A: As seen by the silverwastes introducing 5 easy titles in one update this may be an issue, a possible solution would be limiting the number of easy level titles that count towards the track (could be independent for each aspect of game-play)

I really like this idea. Although I never achieved it, GWAMM was something nice to work towards that really tied in your title achievements. I don’t entirely agree with your difficulty categorisations (though I appreciate they are more demonstrative than anything else).
I also like the colour idea. I think its important to be able to make these titles stand out because GW2 has so many more titles than GW1 ever had.

The real difficulty is finding titles that are epic enough to compete with the Anchorman quotes of GW1 . . .
