that message seems to be glitchy, I got it on my rev, and all my Hom titles were missing, then I logged to a different toon and they were still there, switched back and they were on the rev as well.

It happens every time you make a new character.
Just log in an older character you have and go to the HoM, your achieves will be restored.
I also get that message every time I make my key farm toon

It’s a bug already well know now:
And it will be fixed with the next patch… and there are also some “temporary” ways to restore your achieves.
No panic :P
