Who here holds down right-click at all times?
What I did was set my A and D keys to strafe left and right. This makes it a lot easier to place AoE skills and stay on the move while releasing left click. It helped my game play a lot.

I have too. However, having to let go of right-/left-click everytime you have to use a ground targeted spell means you lose control over both the camera and in which direction you can move (unless you’re aiming to cosplay as a turtle anyways). To me that just feels awkward and I’d rather just not use the spells.
Whenever you use ground targeted spells while holding down right-click, you always aim where your cursor is.
Disable “right-click to attack / interact” in the options.

My mouse is fine, but my middle finger has been feeling out of sorts because of constantly holding down the right mouse button.

I hold down right mouse button a lot yeah, even though I do have A/D bound to strafing rather than turning. But I’ve never had the problem OP is describing. I have fast cast with target AoE and it doesn’t take much effort to release rmb, place the AoE and resume holding rmb.

This is a fab action when you can’t see a dam thing when timing counts. It is also quiet boring- Really – Point- Click target- then Action went out years ago.

It would be nice if it was an option, though. “Shooter Control” On/Off. That way those who don’t wish to use it don’t have to use it, but those who would like it, can use it.

I play Tera (although not hardcore, just a high level mystic), and I plead for this kind of control not to ever make it in GW2. It’s horrible. If you’re used to checking your surroundings while fighting, you’ll just end up aggroing by hitting everything around.
It has it’s place in Tera combat (although the healing with same + a lock on feels just clumsy as hell), but not at all in GW2, in my humble opinion.

There was a pretty complex autohotkey script once that simulated this to the letter (before ESO even came out).
Only problem is that Anets approach to such things are lalalalala scripts dont exist lalalalala dont say a word lalalala they are not allowed lalalalala and proceed to infract people just mentioning that you can macro and script stuff. All it really did was hold down your rigth mouse button and gave you a crosshair dot, the rest was done by changing settings/keys ingame.
Yeah I know where this post is going to end up. I just wish they’d make an official version of the more mouse driven control. Just a checkbox for it, thats all.
This issue is nothing compared to forced angle-snapping upon teleport; that’s just downright disorienting.

Shakes all the time? Do you have Parkinson’s? I have never had a problem with the camera in games like Dragon Nest or TERA.
I really do not see the problem to make the direct targeting appear on the mob you have currently tagged..I still wonder why this is not implemented yet.
I always place the mouse cursor just a little above my head before holding down the right button, that way I know the aoe fields will be going down in front of me. Makes it a lot easier since I don’t release the right mouse much in fights and you can lose the cursor so easily because it blends in when you keep moving it on and off players.

I hold down right click on the mouse very often. It could be that I’m simply used to other mmorpgs using right click to reposition camera. I use normal ground targeting, although it’s slower, it prevents me from accidentally misplacing AoEs. Haven’t had any problems to what is being described. Could be one of the option settings like ‘Right click to Interact’.

Use a mouse with a lot of additonal buttons like the Logitech G600 or Razer Naga and map all the skills on the buttons. Then use the option Fast-Cast Ground Tageting so that you use the skill on release. Its really super fast and you can easily strafe/lock forward to enemy while using such attack skills.

I kept my mouse button 2 pressed all the time and it was getting really tiresome on my hand. Then I downloaded Immersive combat mod (http://wesslen.org/ICM/) and have been using it for months.
Really helps out and makes the game more fun. It doesn’t give you any advantage, but just eases out that you don’t have to press button for mouse look all the time.

TERA i managed at least to play for 2 days .. but it felt really like physical work for me.
Other games like DCUO, Vindictus and Neverwinter with that kind of controls i
deleted right after trying it for 10-15 minutes.
My biggest fear before finally playing GW2 was, that it maybe also had this kind
of controls .. thanks god it still has MMO style controls.
However if its optional i have no problems with it. But all these other games also
don’t have the option for a MMO type of control. I never understood that especially
for Neverwinter, since it uses the same Engine (more or less) than Champions Online
and CO was great to play .. a lot like GW2 already in some cases.

I hold down both mouse buttons all the time while I’m running around (which locks the camera to the mouse and moves you in that direction as well), but when I want to ground target I move with the WASDQE keys until I finish and then I return to the mouse. The only time I actively change how I’m doing this is when I’m running grenades on my engineer, since the grenade skills are exclusively ground-targeting abilities. I do a decent amount of sPvP and WvW as well, and I have no issues controlling like this. I also kept all of the default movement controls on the keyboard.
I don’t know if the “active switching” style would work for you, but it works for me. My hands don’t cramp up from it unless I play for, like, 4 hours straight (which I don’t often get to do anyway).
