losing the mouse sometime in big fights...

I have lost my mouse so many times, but he always turns back up when its cheesetime.
Apparently the mouse code of the game is a mess.
One guy showed up on Reddit saying he had looked into adding a right button toggle because he was tired of holding it down, and found that there was code all over the place checking of the button was down or not.
A workaround if you have Win7 is to enable the feature that puts a white circle around your mouse pointer when you hit CTRL. It blooms, then contracts. When someone first posted that in these forums a red post came in with thanks for pointing it out.
I’ve been using it and it has helped, especially since I mouse-move and thus my cursor goes many crazy places (including to my other monitor at times, though I can tell that happened just by suddenly having my taskbar pop up).

my workaround (Mac): minimize GW2, open System Preferences, Accessibility, Display, Cursor Size. Slide the bar to the right. I settled on about 2x normal size. It will make your computer’s cursor bigger, and that size will carry over into GW2. enjoy!
these steps are slightly different for different versions of Mac OSX, so if you can’t find it, try the search field in System Preferences for ‘cursor size’.

This has happened to me many times, but i finally found out that the suspect was an external HDD i had connected.
From times to times this HDD was awakening, and when it did that mouse disappeared and appeared after some time. Since then i always disconnect it when i launch GW2 and i have never again faced that problem.
Just tried the YOLOmouse. It does work ingame.
If at any point i cant find it, I quickly push into top left corner and my mouse is there in the corner. my Mouse can disappear on the far right side and the bottom of the screen. On a similar note, Anet needs to add " Lock cursor to window"
