This is similar to the Bifrost crystal issue (Bifrost is worse).

So far I have made 3 attempts to enter the watery grave to complete the colossus. You only get to try once every 4 hrs when Teq’s health is <75%. If you are on a zerg map there is little chance of a being taken under. On a dead map teq will not get to 75%.
The other boss requirements that were good are ones that don’t rely on RNG.

The whirlpools are hard to see because they have the same orange ring as the damage and cripple orange rings.
If you run around the zerg and look at the pools you should be able to see a swirl of water in some. I don’t believe they can be cleansed with Turret 3 either but could be wrong.

Same issue as OP.
I’ve been chasing “watery grave” for about 1 week, I experienced the “underwater phase” only once, realized that my buff was not active, triggered my buff, suffered from damage underwater (almost died there) but did not get the collectible anyway.
I can understand that some collectibles may be hard to obtain but that one relies on the other players too hard, I think, and I still do not understand why the buff in watery grave did not triggered the collection either.

I thought the whirlpools auto spawned after the first burn phase, normally the zerg as left the area though so most don’t see them, but im near sure that as soon as the first 25% health is gone in the area around teq when that shockwave goes out the whirlpools spawn.

Yes, it starts spawning the whirlpool once he gets below 75% hp. The problem? They despawn when battery defence phase starts (which is some 1-2 seconds after they appear), and do not spawn during burn phases.
On a good map (which goes from burn straight to next defence), that’s like 2 seconds entry window per whole fight – and if it won’t spawn like 2 steps away from you at most, you won’t be able to make it. On a bad map… well, you likely won’t get it either, because you will not even get to that 75%.

Darkwvlf was right, it is within the orange rings, which can be really tricky because some are poison and some are large AOE damage and a lot are overlapping. You will be lucky to notice a swirl.
I switched to my necro and got some cheap Clerics gear with cheap runes of Dolyak and traited for healing and damage mitigation. You just have to tank out the AOE at the 75% phase and you will get pulled under. Literally just kitten in the AOE – avoid the massive damage ones. I got pulled under twice in one fight.
