Unless the bug has cropped up in the past two months, you are just having bad luck. I got it quickly in August when I was doing that step for Nevermore.

I completed the nevermore collection.IV few days ago. I spent 2 days in the map waiting for the event to spawn. No luck. Is exactly how the OP says:
1. Map is bugged (or the prerequisites for this event became bugged)
2. Never have other IP map – TRUE. The map has a low population, so not enough players to create another copy of the map.
3. Never fix – TRUE. This bug was reported immediately after the launch of HoT – one year ago. It is still not fixed.
To OP: don’t give up. When you login into GW2, try the map – and see the IP. Wait 10-15 minutes for the event to spawn. If the map is bugged, leave it and play the game. Come back next day and do the same. And so on. This may take few days but at least you will spent 15 minutes on the map instead of waiting for hours.
Show up like 5 minutes late to the maw and you’ll likely get a new map.
The event is a part of the ice sculpture chain, so it won’t reset if the other parts are active. The problem is, Ulfred can bug at the end where he asks for ice, causing the chain to block.
