I feel like a lot of people are confused about both Mordremoth fights and how they fit into the story, which fight to do first, etc.
This is a theory
It’s a simple explanation: They both happen at the same time.
This can be supported when Canach says something along the line of “He’s focused on the battle outside.” during the final instance.
During both fights, Mordremoth is basically multi-tasking, trying to kill off the invading Pact members and trying to stop you from exploiting his weakness.
More that supports this theory is how he’s slow on attacking when it comes to the outside (Mouth of Mordremoth), he’s putting most of his focus on the battle inside. I would too if someone was poking my weakness.
One last bit to point out is how he dies outside.
During the final phase in the instance, he is invulnerable as an endless wave of shadows attack you. At this point outside, after being merely annoyed and partially wounded, he begins climbing up his tree…thing. I assume that he is preparing to do some form of devastating attack with the ley energy as his body begins to glow brightly.
The moment you defeat him in his mind, it causes a surge of pain as he continues to climb up, causing him to flinch and become dazed as he accidentally impales himself against his own tree thing, killing the Elder Dragon.
Of course then there’s the whole Trahearne thing that happens last, truly killing him.
Hopefully this has cleared up some thoughts on these questions.
Go ahead and correct me if I appear to be wrong in some parts.

Just to be clear and you may understand this already, but killing the Mouth doesn’t kill “him” or his physical body. It kills the extension of him that acts as his mouth. That could be unique and something he has to re-grow, but killing the Mouth doesn’t equate to killing him.
Otherwise I don;t find much wrong in your interpretation.

That is true, the mouth can just be an extension of his body. But I feel like my theory just…fits. At least for now until someone else makes a theory that’s fitting.
Also, the mouth doesn’t look like something that can regrow (Maybe it can). It may just be called “Mouth of Mordremoth” for being just his head, since all the vines are a part of him and he would be too big to fight his full body like Zhaitan.
Plus, the way it ended, plus the way you end it in the instance. It just fits.
I have a hard time explaining things to people, still, you can correct me.
