Mad King Thorn's return
It was actually explained in some interview a while back for why Thorn disappeared. Basically, the Lunatic Court’s attempts to free him full time backfired, and they’re the group that thinks “if it didn’t work the first time, try it again but harder!” The Lunatic Court’s attempts to break the seals actually made them stronger… for a time.
Don’t remember where this interview was exactly, but it was a video (I think a Live Stream one) with John Stumme (who was the one who answered it).
Stop treating GW2 as a single story. Each Season and expansion should be their own story.
Pretty much, the seals are weakest at that specific time of the year and that’s when he shows up. In GW1 the Lunatic court kept trying (specifically at that time of year) to weaken them to bring him back forever.
Obviously(as Konig said) it backfired, and only then did the seals weaken enough to allow it again. Players/priory fixed the seals.
Instead of freeing the Mad King, the Lunatic Court actually banished him to the Mad Realm by accident, if I remember correctly. And this has made the Mad King a bit ticked off.
The problem is, the Lunatic Court doesn’t really know what it is doing. They are crazy after all. They’ll throw everything at it they can think of, including the kitchen sink.

Why did Mad King Thorn disappear from Tyria for so long only to return on GW2’s first Halloween?
He called me a fruitcake. Lessons had to be taught.
So…….. What additions will appear in the next Halloweens?
Grenth?…… King Frozenwind?….. Bruce the Herald?….. Vale Wraiths?………. A Jester?…… The Teller of Tales?………. Lunatic Courtiers with flaming pumpkin heads(unlike the ones with normal heads that appeared during GW2’s first 2 Halloweens)????

All of the above is very unlikely, Grenth and his forces have left Tyria much like the other god’s have, although remnants of them remain. Like me.
The Teller of Tales was elusive even in GW1, did you spot him?
Lunatic Courtiers could possibly appear, although my bets are on Mad King Thorn’s many wives appearing, as well as The Bloody Prince.
I also hope to find out more about Thorn’s Palace. (The one me and Malafide stumbled across last Halloween.)
Bloody Prince Thorn has already appeared and is likely still at war with Mad King Thorn so for the story to be fresh we need particular additions.
The Mad King’s castle or his wives(last I heard Mad Queen Malafide was one) maybe?
Grenth isn’t in Tyria yet he must be somewhere…. He is the ruler of the Underworld which the Mad Realm is apart of……. Why shouldn’t he be vacationing there when not doing paperwork? Infact he’s the most likely God we’ll ever get to see.
Grenth is in the mists, likely in his house in the underworld.
And the Mad Realm is also in the Underworld….. What are the chances that Grenth won’t get bored enough to spend a day in the Mad Realm as Mad King Thorn’s guest?
If anything Mad King Thorn is tolerated in the Underworld by Grenth as his “guest”.
I theorize it’s only because Mad King Thorn keeps the wild invisible ponies in check.
Either way it is technically possible for Grenth to make an appearence in the Mad Realm since it’s part of the Underworld where Grenth resides.
Considering that the Dhuum encounter was introduced by Halloween it shouldn’t be a problem to have Grenth himself make an appearence in the Mad Realm without ever visiting Tyria.
By the way are we even sure that castle is Mad King Thorn’s castle? It could be Grenth’s or even someone else’s castle….. The Mad Realm is in the Underworld…..

Grenth has more on his plate than a mad spirit lord who’s own minions (as Konig rightfully said) sealed him up even better than he had ever been sealed before.
Grenth could visit the Mad Realm, but he has no need.
Especially as we don’t know how the Battle of the Underworld has fared in the past 250 years.
Dhuum is a more pressing concern, and Mad King Thorn knows it.
The Mad Realm maybe in the Underworld, but it’s just that, mad.
The fundamental physics and laws of Tyria don’t really apply there, so the palace could be anybodies.
However!
When the question was posed on twitch to Anet, they became very tight lipped.

According to Priestess Rhie (who acts like she is speaking for the general knowledge of the human priesthood), states that it is “known” that the Six are not in the Mists. And we know they’re not in Tyria.
So the gods are most likely in another world. Which excludes the Underworld from possibilities, given that it is in the Mists. Even in GW1, the gods were not in their realms.
Grenth showing up for Halloween is extremely unlikely given ArenaNet’s actions on the Six thus far in GW2.
What is most likely is a mere expansion of Eddie and Ozzy’s conflict, since that was the plot element of Halloween 2014’s plot, even without Eddie’s presence. No need for new faces, though some returning ones wouldn’t be bad (I’m looking at you, Dhuumy-boy).

Actually, there is an interview statement which says the gods ARE in the mists.
I’d have to dig it up, but it does stay that the Gods are in the mists, and simply stopped interfering with mortal affairs, especially since Abaddon died.
I think you’re recalling the Dolyak Express Q&A in which Jeff Grubb responded to a question about the gods, saying thus:
Jeff Grubb: The human gods still exist, and their power is still felt within Tyria. However, they have pulled back into the mists, leaving the humans to stand (or fall) on their own merits. There has been a tendency for the human gods to, um, meddle with their worshippers a bit much, and in the wake of the final battle of Abaddon, they have been trying to cut back. Also, the destruction of the big A and his replacement with Kormir in the Pantheon resolved one of their ties with physical contact with Tyria. So there are ties, but you just can’t ring them up to take on the Elder Dragons.
In all honesty, it’s hard to say which can be more accurate because, well, EVERYTHING is in the Mists. So to say something is or isn’t in the Mists can mean a) it is/isn’t in the space between worlds, b) it is/isn’t in the afterlives, or c) it is/isn’t in Tyria.
Honestly, I think Jeff is referring to the last, while Rhie refers to the first or second (given her “they aren’t in the Mists, that’s for sure” kind of explanation).

If no humans have been in the underworld for a while, who confirmed that?
