I was wondering if it were possible for someone who has died to come back from the mists somehow. Not as a ghost but flesh and blood.

As far as we have seen, maybe.

People can come back from the dead, but no one has done so while in the Mists, and all but one success and two attempts required one’s original body. That one success exclusion would be Shiro Tagachi (the two failures would be Mad King Thorn and Gaheron Baelfire), but such was not in the Mists though there is no reason why what Shiro did could not also be done in the Mists. But normal resurrection hasn’t worked since around gw1’s time for reasons unknown.

It’s possible- it was actually a major part of Factions- but it wasn’t something that came easy. Shiro had to ravage a continent to search out the knowledge and ingredients to cast the spell, which included killing a member of the royal blood, and he was an Envoy- it may be that such status was a prerequisite to make the spell work, and if nothing else it’s unlikely that he’d have had the freedom to find what he needed if he’d been a normal ghost.

Like Konig mentioned, though, it’s something that’s a lot easier to do with A.) the aid of the living and B.) the presence of the original body. The one case we see in GW2, Gaheron, may well have succeeded if we hadn’t intervened, and he had the former and possibly the latter.

Sadly, I never play GW1 so my knowledge about that lore is incomplete. However, I think that all depends of how you enter in the Mist , basically if you enter in the mist without died (Rytlock) would be a lot easier compere if you actually died and you soul is send to the Underworld. The Underworld is a realm inside the Mist, hence there is a difference between leave the underworld and leave the mist, even if this concept is sometimes mention as the same and obviously one realm is inside the other , The Mist is more like a dimensional collider, the fabric of space and time on GW universe.

There is still the gw1 ritualist hero, Razah. He literally came from the mists.

Razah is a being created in the mists, like demons are, not a mortal returning from the mists after dying which is what OP was asking for.
