After defeating the steam ogre, I went to interact with the chaos energy to infuse my chaos orb for the back piece… and immediately died. In addition, no one else around me was able to revive me.

I haven’t had a chance to try again, but just throwing this out there in case it’s happened to anyone else or it’s a bug.

Lag-induced death. When you’re on an elevated platform, you are at risk of the server re-establishing your position at ground level, which causes the physics engine to assume you fell the distance. The result is usually instantaneous and inexplicable death, plus the confusion between where your client and where the server thinks you are makes players unable to interact with your corpse.

The platform kills anyone on it a set amount of time after the Steam Ogre is defeated.

However you do mention that no one was able to revive you – so what Sariel V said is probably the case here. It’s a disconnect with the client and server, they don’t see your corpse as where you see it or vice versa.
