100% Map Completion
You probably completed a map before a revamp. That map will show 100% but the revamped points of interest will be undiscovered.
Check Kesses Hills, everything around the tower of nightmares and the lake. Check also Lornar near the Middle-East side, the ground of the marionette.
It will also help you to open the big map and hover over the name of the maps, it will show you if you miss something in any map.

You’re both right actually. I never knew about the Marionette and have a few spots in WvW. Completely forgot about WvW. Thanks guys!
If each map is at 100%, then the missing POI are likely added since the game launched. Check the wiki; I think it has a list of those.
There’s no way I can think of that would allow you to have missing skill points and vistas and have 100% on each map. Could you be missing 100% in WvW?
If not, I’d recommend visiting each map (not just hovering over) and looking at the map completion status until you find the maps with gaps. Alternatively, you can wait for the next update, which supposedly includes hints in the UI to point you in the right directions.
