100% map but still 5 PoI
Usually these things are entrances to JPs and mini dungeons in open world you haven’t seen. When I was looking to finish this, I used the wiki for the maps and pored over them to see which white lettered region I was missing. Another thing you can do is look for blurry spots on the map. Sometimes they’re just small areas you missed while traveling.
These are the things that pop up on your screen “x place was discovered” and the region marker name is then placed on your map. For maguuma, my missing regions were in caledon, and for shiverpeaks, timberline and dredgehaunt.

They added new POIs for several living world stories, in Lions Arch, Divinity Reach, Kessix etc. Also the POIs from guild jumping puzzles.

I also tried looking throu the patch notes of the last 2 years for all the stuff added in the map, but already have all of them i think…

I think that since it says you already have the PoIs, you’re fine on that front. The Explorer achievements are different. They have to do with the white lettered regions on a map (like the skritt cave in metrica).

Those explorer achievements are related to the POI’s. You may remember that when you run into a new region a message pops up with “such and such discovered” and you got some XP, but no POI’s were shown. This is what that explorer title relates to. There are some regions inside caves which have no POI’s but are counted as zones. Check the wiki maps. You’ll see that each map is sectioned off and have names. If you compare those names to your revealed world map you should notice some areas that are still hidden under fog. Just go check for cave or underwater entrances in those areas

Google Maguuma Explorer achievement and Shiverpeak Exlorer to find lists of commonly missed areas for those achievements. They are zones in the map with no WP or POI or heart or skill point.

This post I made a while ago may help you, the first gives some hints and tips for tracking down missing explorer areas and the second post lists the ones people commonly miss as they contain no other items necessary for map completion.
Just to clarify, your explorer achievements are not connected to 100% map completion. Although for the most part the two go hand in hand, your explorer achievement counts the number of areas (named in white) you have entered on your map, but these areas themselves are not counted in 100% map completion, just some of the things they contain (waypoints, poi, vista, skill point, etc).
