Have you revisted the chantry of secrets AFTER you did everything else??. The game tends to forget you have been there.

Did you get Forsaken Halls in Dredgehaunt Cliffs? That was one I missed when I was doing them.

There is no PoI in forsaken halls and forsaken halls have not to be visited for 100% map completion. It is only needed for the unrelated shiverpeak explorer achievement (unfogging all area’s i nthe shiverpeaks)

You dont need newly added POI’s if you got the map completed before the POI was added.

The change that he actually miss the chantry of secrets one wich is bugged or any other he missed going round is higher then missing the one added last. I appreciate it your trying to help, but your now only making it confusing, there is lesser change he is missing that one then he is missing a PoI that is allready in the game. The reason is that there is a very high change the OP had 100% on divinity reach before it was added and so he doesnt need it at all.
