2. at least ascended trinkets and weapons, stats depend on the class you play and role of that class, food and utility is a must.
3. it depends on your skill and skill of other 9 ppl. It can take 10 min to 9 hours per boss.

1. Entrance is in Verdant Brink, LFG or find a Guild that does it.
2.& 3. Dulfy.net has guides that are fairly good. Do not use it as the only way to do things but a good overall view.
