I have 14/17 achievements from Aetherpath. But how do I get Broken Clockwork?

It says: “Defeat the Clockwork Nightmare during the Twilight Assault”

What does this mean?

Did it mean beat it during living story? I did… I got the backpiece too.

So how do I get it?

IIRC you get it by following Caithe at the very end, the nightmare will pop as you open the chest in the room.

Yep, you have to follow Caithe into the secret room, wait until her dialogue has finished and then you can open a chest to trigger the clockwork nightmare.
