Can't get 100% of other races' starter areas
Never had this problem. Try GW2 wiki for help. That’s how I found a lot of missing POIs, vistas, and hearts.

I have 100% in every starting area multiple times.
A tip for getting the last heart (trust me, there is ALWAYS one missing heart at the end of every area :P ) is to look for the Scout icon on your map. Scouts only appear on the map when you have not done/found the hearts they tell you about. If there is one missing heart, there must be a Scout who can point you in the right direction.
Some hearts are hard to see on the map unless you scroll in all the way. There is one in Queensdale like that, and a few in the higher level maps, but I can’t remember if there is a heart like that in Caledon Forest or the Plains of Ashford.
Note that some can also be underground, so be sure to check all the layers of a map. Also, be sure to use the scouts, as they will put unfilled hearts on the map even on undiscovered regions.
The first part of your storyline is not a heart on its own, it just requires you to complete one heart in your starting area. It can be any heart in that region.
100% map completion is definitely possible for the entire world, for any race.

The problems I have are not hearts but trying to get skill points working, im missing one in the Charr area.
I haven’t yet completed Charr starting area, but for Sylvari, the single most common heart to overlook is the Ogham Wilds heart. I don’t use the scouts, personally, I’d rather find my own way — so I don’t know which scout would show it. But the heart is south of the skill challenge at the waterfall, with patrolling breeze riders. Fastest way to get there is to exit the Grove, and take a left rather than go straight ahead to Astorea.
I’d also recommend checking the wiki. I can’t paste the link into the edit box here (stupid forum), but search for “renown heart” and at the bottom of the page there’s a list of hearts by region.

They’re still in the world and say that when you talk to them, but their icon no longer shows on the map.

I’ve spent a lot of time in Caledon forest, first for my characters, then later with my wife and kids. People are always looking for one last heart, and this is almost always it. I want to say that 95% of the time, it is this heart, the other 5% is the small nightmare court outpost on the east side, near the skill challenge.

In the Plains of Ashford, there is a Renown Heart all the way to the southeast (to help an Asuran scientist) and a skill point along the south edge of the map south of Lake Adorea, that may be tricky to locate.

I’m always missing a heart or a point of interest or whatever no matter what map I’m on and the site posted and other sources I’ve found via Google really helped me figure it out as I can visually see what I’ve missed. Very helpful link!
My main character is a Slyvari and I had missed the one where you turn into a “tree thing” and you stomp things…etc. I can’t remember the name of it, but it was tucked away in a corner area of the map that I hadn’t been too apparently.
