Where are these coming from?
I am level 70 and have done it all (PVP, WV3, PVE, completed zones..), but I have never been presented with the option to have a flaming sword skin or even shoulders.
All of which I have seen on other players.

Where do I get these most awesome items?!? ‘O_o’

I think you’re thinking of the hall of monument rewards.

You might also want to check out the rewards for the Cathedral of Flames dungeon (I think that’s the one). You can find the reward merchant in Lions Arch near the WvW portals and preview the armor there.

Atom the sword if 1 handed is a hall item if the gloves was as well same again…a hall item.
As to shoulders there is no flaming shoulders in the hall list of items so that will be a in game farm-able item.
Best bet is to go to the mist and go to the pvp bank and pre-view the armours and weapons in there to see the rough name of the item you want. Then ask players if they know good place to farm or how to get them. But I believe there is more item skins in pve than in that list but it will help.
