Items breaking
If you die enough that the x item (discounting weapons) breaks (goes red/broken shield icon) all you have to do is go for a repair vendor/anvil it your good to go again.

You can always repair your gear.
“Breaking” goes in two steps: Every time you are killed, one of your armor pieces (including back items) starts to break, getting an orange symbol to show it.
Once all worn armor (and back item) pieces started breaking, the next time you are killed one of them breaks down totally, getting a red icon to show this. Broken pieces (the ones with a red icon) will no longer contribute their stats, runes etc. to your character, until you get them repaired. Once repaired, they are back to normal, as if nothing ever happened to them.

Repair is free, by the way. Don’t repeatedly repair since you can take a total of 7 deaths before anything goes red, but it’s good to ensure you’re in top condition before you start a dungeon, even though the chances of death in dungeons nowadays is nearly zero.
Several dungeons have anvils at the start point for this purpose.
There is also an achievement for breaking armor in the Hero Achievement under Hint Completion….
