Mad king and bloody prince
The Bloody Prince is in an instance near the Nolan Academy Ruins Waypoint in the Black Citadel (the Charr city). There is an NPC east of there with a gold star above his head. You speak to him.
The Mad King, I’m not positive, but I think he appears at the top of the hour.

We can’t. The mad king fight was a mini dungeon that was in the 2012 halloween event and hasn’t returned since then
