The fourth part of my monthly simply reads ???. Is it something I have to unlock? Hard to imagine that across all my characters there’s something that I haven’t done.

It’s speculated to be associated with the event coming up for Halloween.

It was the slightly bugged achievement for monthly survivor in that spot. It has been disabled since the first day off the months. It is unknown if it is disabled cause of the bug, if they are planning to bring it on later in the month or if it is going to be replaced by a one time event achievement (and it is also unknown if that is to cover up for the bug or if it was planned on all along). We will know when halloween events are starting…..
