These hearts are the most easily missed in the area:
> Akk Wilds, southeastern corner, helping out with the gate research.
> Cuatl area, ENE of Desider Atum — deal with the hylek.
> The Anthill – deal with skritt and chaos poisoning w/ the Chaos krewe.
Compare your current map with the one in the Wiki. Also, hover over the 15/16 while in Metrica and you might see the one you missed glimmer. Check for blurred spots, too.

If you open your world map and zoom out a little bit, then move the curser over the heart counter in the upper left corner, it should flash a halo around one of the eyeglass “scouts” you can talk to to reveal the heart you need.

Neat new one for me – four years playing!
Another feature that is not some obvious is that when your overall world % complete gets high enough you get a screen pointer in the top right corner to the nearest missing point on the current map – not always useful but helps. Think the trigger is above 90% complete.
Another, maybe faster, method is to set options – content guide to “hide events and personal story”. It’ll point you to the closest poi/vista/heart you’re missing on that map.

This is the method I always use; F11 (game menu), change “Default” dropdown to “Disable …”
Points to closest hearts, POI, waypoints, events and undiscovered areas of the map.
I think there’s usually a Scout, that’s highlighted, near the missing heart. If you’re roughly in the middle of the zone, he should show up on your compass. Go to him/her and talk, and that will make the heart(s) appear.
