You only get the items from the last time you transmuted it.

The game doesn’t keep records of every transmutation you’ve done. It just matches up the current parts of the item (the skin and the stats) with the default ones and gives you back those.

So you’ll get back the item you’re currently using the stats from, with it’s original skin. Plus either the item you used the skin from, or (if it was a skin item that has no stats) that skin applied to a random blue back piece.
