all about ascended
I suggest you start by reading this page: http://wiki.guildwars2.com/wiki/Ascended Maybe also read some of the pages it links to which also have useful information.
You can’t craft all the Ascended items, they come from a lot of different places like doing daily and monthly achievements or guild missions, or Fractals.

infused back + 5 AR infusion
infused ring + 5 AR infusion
infused ring + 5 AR infusion
non-infused amulet + 5 AR infusion
non-infused accessory + 5 AR infusion
non-infused accessory + 5 AR infusion
= 45 AR
amulet and accessories are not infusable in the MF yet (or ever? who knows?)

Taken from this wiki page: http://wiki.guildwars2.com/wiki/Infused_equipment
Upgrade a regular ascended ring in the Mystic Forge using 1 Shard of Crystallized Mists Essence, 3 Globs of Coagulated Mists Essence, and 5 Vials of Condensed Mists Essence.
Upgrade a regular ascended back item in the Mystic Forge using a Glob of Coagulated Mists Essence, 250 Globs of Ectoplasm and a Gift of Ascension.

it depends on which back piece to infuse it. something along the lines of: non-infused back + gift of ascension + 1 shard of mist essence + 250 globs of ecto
to infuse a ring: non-infused ring + 1 shard of mist essence + 3 globs of mist essence + 5 vials of mist essence
when you infuse a back/ring, any infusion that was in it is removed. So:
non-infused ring with +5 AR infusion into the MF to infuse it = infused with with +5 AR and empty infusion slot. then if you add another +5 AR infusion to it, the ring will have a total of 10 AR.
