noob question: red flag over hostile NPC?

Q:

Some hostile NPCs have red flags over their heads; perhaps this only occurs in dynamic events. (The flag is a red square with something white crossing like a St. Andrew’s Cross.)

What does the flag mean? I’ve assumed that it indicates an NPC that needs killing.

I always thought it was crossed swords… and believe it helps show what NPC’s have aggro’d you. I tend to see it a lot when they run past allies in front of me when I burn the hell out of them and they decide to hunt me down.

It does seem to me they appear when you hit their friend and they decide to attack you as well. It is indeed crossed swords so perhaps it is simply to let you know they are attacking, because espeacailly with rangeed mobs you might not have noticed.
