:/ chrono has better survivability than mirage and other than jaunt all of the teleports are toward your target, with mirage advance not being useable without line of sight.
And if you don’t count stealth thief is artificially way to low.
If you want to quantify something attempt to look at how safe a class is to achieve their goal or damage, and include their follow up.

Barrier and Shroud probably shouldn’t be calculated as though they were blocks/dodges/evades simply because they can only take so much damage whereas other active defenses can mitigate an infinite amount. If anything, they should be calculated as healing.

A nice effort. Teleports are far more valuable than you have listed, though, since they can allow you to get to LoS or disengage completely to recover for a far longer duration than 66% of a dodge. There is also the question of healing, protection, stability and stun breaking. CC can also be used defensively.
You have also missed some teleports. Necro’s flesh wurm and spectral walk, thief’s infiltrator’s signet, infiltrator’s arrow and infiltrator’s return, mesmer’s phase retreat and portal, and revenant’s.. I forgot what it’s called, but it’s the assassin’s stance teleport toward the target. While teleporting toward a target may not seem like a survival skill, you’ll see skilled players use it so quite often, because it gets you out of or away from something or someone. Especially if you have a target far away from the fight, you can port toward them, but not "to" them, and be clear of any harm.
Additionally, this is only for direct damage. There is no mention of resistance or condition cleansing skills.
To be more accurate, you have to be far more general. i.e. less accurate, ironically.

3000 Barrier = Evade Duration 1
3 Blocks = Evade Duration 1
50 Endurance = Evade Duration 0.75
1 Teleport or 1 Shadowstep = Evade Duration 0.5
(Stealth Duration 2 = Evade Duration 1 ) but I took out stealth from the list
Barrier is not equal to an evade, not at all, not even close. You get hit in a 3k barrier by a 15k gunflame it’s still going to take 12k health off you, evade it does 0 damage. You shouldn’t be factoring in barrier without protection and the calculations would get really weird at that point.
